-The midwife in the operating room: the control of deadlines and the supply of sterile material
- the concept of sterility and asepsis in the operating room
PRE-OPERATIVE PHASE
- reception of the operanda in the preparation room
- verification of the identity of the operanda
- measurement and recording of vital parameters
- checking the completeness of the medical record (exams, anesthesiological and operating records, informed consent)
- information for the woman relating to the intervention
Prep aration of the operand:
fasting from midnight
trichotomy and evacuative enema
deep vein thrombosis prophylaxis (elastic bandaging of lower limbs)
possible premedication if requested by the anesthetist
placement of a peripheral venous access
application of electrodes for intraoperative monitoring of cardiac activity
insertion of the bladder catheter
INTRA-OPERATIVE PHASE
- correct positioning of the operanda on the operating bed (abdominal, vaginal, endoscopic surgery)
- neutral plate application for electrosurgery
- technique of: surgical hand washing, dressing, sterile gloves
- preparation of the sterile table and the operating field
- preservation of sterility
- preoperative control and counting of instruments, gauze, compresses, needles
- elimination of needles and other sharps used in the surgical field
- laparotomy wound dressing
- Instrumentation of the main abdominal and vaginal surgeries in obstetrics and gynecology (main table and servant table, surgical times from the point of view of the instrument nurse):
1) abs:
conservative cesarean section
laparohysterectomy with bilateral preservation of the adnexa
laparohysterectomy with bilateral adnexectomy
demolishing caesarean section
microsurgery and tubal sterilization
hints of laparoscopy
hints of gynecological robotic surgery
2) vaginal.
instrumental revision of the uterine cavity
cervico-segmentary cerclage
small vaginal sutures
vaginal hysterectomy with and without bilateral adnexectomy
POST-OPERATIVE PHASE
Immediate post-operative assistance in the recovery area of the operating block
Control and registration of:
vital parameters
state of consciousness-vigilance
quality and extent of diuresis and any drainage
infusion therapy and patent venous access
- transfer to the hospitalization ward (stretcher with safety rails) after anesthesiological check-up
- control of pain symptoms (FUT)
- early mobilization and/or mobilization exercises of the lower limbs
- resumption of therapy for thromboembolic prophylaxis
- oral and personal hygiene and cleaning
- control and replacement of the dressing
- control of the resumption of normal bowel and urinary function
- care of the feeding of the opera
- psycho-emotional support of the work
In the field of pelvic floor functions and dysfunctions, primary, secondary and tertiary prevention and obstetric care
Prevention, education and rehabilitation of the pelvic floor during pregnancy, childbirth, the puerperium and pelvic static pathologies
